When should a parent request an IEP meeting? Parents can request an IEP meeting as many times as is needed through out the school year.
How often and when should an IEP team meet?
The IEP team is required to review the child's Individualized Education Program (IEP) at least once a year to assess progress towards annual goals and revise the plan as needed. This review should consider the child's achievements, any lack of progress, reevaluation results, parental input, anticipated needs, and other relevant information. In addition, as per Arizona State Board of Education regulations, a parent or public education agency can request a review of the IEP by sending a written request to the school, specifying the reasons for the review. This review should be conducted within 45 school days of the school receiving the request at a mutually agreed upon date and time.

Dear School Psychologist Name,
I am writing to request an IEP review meeting. I would like to discuss making some changes in (child’s name) IEP. I am concerned about his/her academics in the following areas; (be specific to your child’s areas of need) reading fluency, decoding, comprehension, math, and writing. His/Her social skills are severely impacting his/her ability to learn in class and his ability to create and maintain positive peer relationships continues to concern me. I also see (child’s name) behavior impacting his ability to learn in school. Note: list examples of incidents
I would like to have the Principal, Class Teacher, Special Education Teacher, Occupational Therapist (include only if you child receives OT in school) , and Speech Pathologist (include only if you child receives Speech services in school) attend. I think their ideas about the changes we may need to make will be valuable.
I can arrange to meet with you and the other members of the IEP team after school. If you can provide me with 3 days and times when everyone is available this would allow me to arrange the meeting around my work schedule. Email is best for me and I will keep an eye out for your response.
I look forward to hearing from you soon.
Thank you for your help.
Sincerely,
